Abstract

Introduction: Oropouche fever was first identified in Vega de Oropouche in 1955 and has caused outbreaks in the Americas, with over 500,000 cases to date.

Objective: To describe a case of Oropouche virus encephalitis and raise awareness regarding this potential complication during the course of the disease.

Clinical case report: A 19-year-old male patient with a history of arterial hypertension presented with symptoms such as headache, fever, arthralgia, and myalgia. Following an asymptomatic period, he developed spontaneous horizontal nystagmus, generalized myoclonus, a positive Romberg sign (non-lateralizing), and deep tendon hyperreflexia, suggesting a neurological condition. Microbiological studies confirmed Oropouche fever encephalitis. The patient achieved full recovery in less than a month.

Conclusions: This case highlights the importance of early diagnosis of the rare but significant neurological manifestations of Oropouche fever to ensure better medical care.
