Cutaneous-Visceral Loxoscelism in a Young Adult

Introduction: Bites from spiders of the genus Loxosceles represent a public health issue in regions of South America. In Peru, such bites occur most frequently in households along the coast and present rapid onset; in cases of cutaneous-visceral loxoscelism, the condition can even be fatal.
Clinical case report: We reported the case of a male adult patient with presumptive diagnosis of a spider bite. The clinical presentation was characterized by chest pain, phlebitis, fever, nausea, vomiting, hematuria, and respiratory distress, with evident systemic involvement that led to the patient's death.
Conclusions: The cutaneous-visceral form of loxoscelism is the most severe; mortality is very high if medical attention is not prompt. Cases of loxoscelism can occur at any age and across all socioeconomic backgrounds; therefore, prevention campaigns should be promoted to reduce the risk of exposure.
